You don't need a letter from your GP to see our allergy team. Send us your allergy history, and our doctors will review it and let you know what happens next.
Send us your allergy history
Complete the ASCIA Clinical History Form — you can type into it on screen, save it, and email it to us.
Open the ASCIA form ↗Email it to us
We review it and contact you
Once our doctors have reviewed your form, we'll email or call you within 7 business days with next steps. That may be an appointment with our allergy team, or other advice if our service isn't the right fit.
What to include in your email
- Your completed ASCIA Clinical History Form
- Full name, date of birth and phone number
- Your usual GP and clinic, so we can send a report
- Previous allergy test results or specialist letters
- Photos of any reactions
If we book you in

We'll ask you to complete the Vida form matching your condition, at least 7 days before. See the forms →
We'll email advice on whether to pause antihistamines or other medicines before testing.
